Does Sugar Age Your Skin? A Plastic Surgeon Explains Glycation, Collagen & Skin Aging

Does sugar cause wrinkles or damage collagen? Board-certified plastic surgeon Michele Koo, MD, FACS explains glycation, AGEs, acne, and how diet affects skin aging.

Yes. Consistently high sugar intake can contribute to skin aging through a process called glycation, in which sugars bind to proteins such as collagen and form advanced glycation end products, or AGEs. Over time, glycation can make collagen stiffer and less functional and may contribute to loss of elasticity, inflammation and visible skin aging.

But that does not mean sugar is literally toxic, that eating one dessert causes wrinkles, or that you should eliminate fruit.

I'm Michele Koo, MD, FACS — a board-certified plastic surgeon and the founder and formulator of Dr. Koo Private Practice Skincare. I have spent more than 35 years looking at skin from both sides: what patients see on the surface, and what I see beneath it.

Skin is living tissue. It is continually building and remodeling collagen, maintaining its barrier, managing inflammation and repairing damage from ultraviolet light, environmental exposure and normal aging. What you eat becomes part of that biologic environment. That is why the relationship between sugar and skin aging deserves more than a social-media sound bite.

How does sugar age your skin?

Sugar can contribute to skin aging primarily through glycation. Glycation is a non-enzymatic chemical reaction in which sugars such as glucose and fructose bind to proteins, fats or nucleic acids. Over time, these reactions can produce compounds called advanced glycation end products, or AGEs.

In skin, collagen is particularly vulnerable. Collagen is one of the major structural proteins of the dermis and is relatively long-lived. Unlike surface skin cells that are continually shed and replaced, some collagen remains within your tissues for years. That gives glycation time to alter it.

When AGEs accumulate within collagen, they can create abnormal cross-links between collagen fibers. Those cross-links can make collagen:

  • stiffer
  • less flexible
  • less resilient
  • more difficult for the body to remodel
  • more vulnerable to oxidative and inflammatory damage

This is one reason skin aging is not simply about how much collagen you have. It is also about the quality and organization of that collagen.

What is skin glycation?

Skin glycation occurs when excess sugars bind to structural proteins such as collagen and elastin and eventually form advanced glycation end products.

Healthy collagen should provide strength while retaining flexibility. Think of youthful collagen as an organized support network beneath the skin. Those fibers interact with elastin, water, fibroblasts and the extracellular matrix to help skin remain firm yet resilient. Glycation alters that architecture. As AGE-related cross-links accumulate, collagen can become increasingly rigid.

Over time, that may contribute to:

  • decreased skin elasticity
  • increased stiffness
  • loss of resilience
  • altered collagen organization
  • oxidative stress
  • inflammatory signaling
  • visible fine lines and wrinkles

This is why I think of skin aging as a tissue-quality problem, not simply a wrinkle problem.

Does sugar cause wrinkles?

Not directly or immediately. Eating sugar does not instantly cause wrinkles. Skin aging is cumulative, and glycation develops over time.

Your body is designed to use glucose. Glucose is an important source of cellular energy, and carbohydrates are part of a normal human diet. The concern is not an occasional dessert. The concern is repeated high glycemic exposure and chronically elevated blood glucose over years.

The higher and more persistent the glucose exposure, the more opportunity there is for glycation reactions to occur. Glycation also happens naturally as we age, and excess metabolic stress can accelerate a process that is already occurring. That distinction matters.

Should you stop eating fruit to prevent skin aging?

No. Whole fruit should not be equated with added sugar or highly refined carbohydrates. Whole fruit contains naturally occurring sugars along with:

Those components affect digestion, absorption and metabolic response. A handful of berries is metabolically very different from drinking a large sweetened beverage containing a concentrated amount of rapidly absorbable sugar.

I would never tell a healthy patient to stop eating blueberries because they contain fructose. I would pay much closer attention to added sugars, sugar-sweetened beverages, refined carbohydrates and the overall glycemic pattern of the diet.

Does sugar cause acne?

Sugar is not the sole cause of acne, but in some acne-prone individuals, high-glycemic diets may worsen acne by increasing insulin and insulin-like growth factor 1, or IGF-1, signaling.

Rapidly absorbed carbohydrates can raise insulin levels and influence IGF-1 pathways. These signals affect processes involved in acne, including:

  • sebaceous gland activity
  • sebum production
  • follicular keratinization
  • cellular growth signaling
  • inflammation

That does not mean sugar is the sole cause of acne. Acne is multifactorial: genetics, hormones, sebum, inflammation, follicular behavior, microorganisms and skincare all contribute. But if a patient with persistent acne consumes large quantities of sweetened beverages and highly refined carbohydrates, I consider dietary pattern part of the clinical picture. Skin rarely responds to only one variable.

How are glycation, inflammation, and oxidative stress connected?

Glycation does more than change the physical structure of collagen. Advanced glycation end products can also participate in cellular signaling that promotes oxidative stress and inflammation.

That matters because oxidative stress can damage:

  • collagen
  • elastin
  • cellular membranes
  • proteins
  • DNA
  • normal repair mechanisms

And skin rarely encounters one form of damage in isolation. Ultraviolet exposure adds oxidative stress. Smoking adds oxidative stress. Air pollution can add oxidative stress. Metabolic dysfunction can add oxidative stress. Chronic inflammation can impair the environment in which normal repair must occur.

This cumulative biology is why I have never believed that healthy skin comes from finding one miracle ingredient. Healthy skin comes from reducing unnecessary injury while supporting the tissue's ability to repair itself.

Can skincare reverse glycation?

No. No topical skincare product has been proven to simply erase established glycation or break all AGE cross-links in human dermal collagen.

That is an important distinction. No serum can compensate for years of metabolic dysfunction, and I would be skeptical of a cosmetic product promising to "de-glycate" collagen as though damaged protein could simply be switched back to its original state.

What evidence-based skincare can do is support the biologic processes required for healthier skin. My goals in skincare are to:

  • protect collagen from additional oxidative damage
  • support normal collagen production
  • encourage appropriate cellular turnover
  • preserve the skin barrier
  • reduce unnecessary inflammation
  • maintain hydration
  • optimize the environment in which skin repairs itself

That is a very different claim, and a much more scientifically defensible one.

Why antioxidants matter for collagen

Antioxidants help protect skin from oxidative stress, which contributes to collagen damage and premature aging. Vitamin C is particularly important because it also participates in normal collagen synthesis.

But simply seeing "Vitamin C" on an ingredient label tells you very little. The molecular form, stability, concentration and delivery system all matter. This is why, when I formulate skincare, I think in terms of chemistry rather than ingredient buzzwords. In my own formulations, I use lipid-soluble Tetrahexyldecyl Ascorbate, or THD-Ascorbate, as my preferred Vitamin C derivative. Your skin responds to molecules, concentration and delivery, not marketing language.

Why retinol matters for aging skin

Retinoids remain among the most studied topical ingredients for improving signs of photoaging and supporting collagen remodeling. Retinol can help normalize cellular turnover and influence pathways involved in collagen production.

But stronger is not always better. Persistent redness, inflammation and barrier disruption are not signs that skincare is "working harder." My approach is progressive: I prefer patients to use the appropriate retinol strength consistently rather than repeatedly injuring the skin with a concentration they cannot tolerate. Healthy remodeling requires a healthy environment.

Why the skin barrier still matters

The skin barrier is sometimes treated as a superficial issue. It is not. An impaired barrier increases transepidermal water loss and makes skin more susceptible to irritation and inflammation.

The epidermis needs an appropriate balance of ceramides, cholesterol, fatty acids and water to function properly. This is why I think about surface barrier health and deeper dermal biology together. You cannot optimize what is happening beneath the skin while repeatedly traumatizing what is happening on top of it.

What should you eat for healthier-aging skin?

You do not need a "sugar detox." You do not need to eliminate every dessert. And you do not need to fear fruit. Instead, focus on the dietary pattern you repeat most of the time. For healthier skin and overall metabolic health:

  • eat vegetables and whole fruits regularly
  • consume adequate protein
  • include healthy fats
  • choose minimally processed carbohydrates
  • emphasize fiber
  • reduce added sugars
  • limit sugar-sweetened beverages
  • reduce highly refined carbohydrates
  • exercise consistently
  • avoid smoking
  • protect your skin from ultraviolet damage

Pairing carbohydrates with protein, fat and fiber can also moderate how rapidly glucose enters the bloodstream. And remember: skin aging happens over decades, not days. The same is true of skin preservation.

How much added sugar is too much?

Current American Heart Association guidance recommends limiting added sugar to approximately 25 grams per day for most women and 36 grams per day for most men.

That is added sugar. It does not mean the naturally occurring sugar within an apple, strawberry or blueberry should be treated the same way as a soda, candy or heavily sweetened processed food. The metabolic context matters.

A plastic surgeon's view of skin aging

When most people look in the mirror, they see lines, pigment, texture, pores and luminosity. When I look at aging skin, I am also thinking about what is happening beneath the surface: collagen architecture, elastin, water, lipids, fibroblast activity, inflammatory signaling, oxidative stress, pigment production, cellular turnover, blood supply, and the cumulative biologic environment in which all of those systems have been functioning for decades.

That is why sugar matters. Not because you need another food to fear, but because skin health reflects the biologic environment in which your skin is asked to maintain and repair itself.

You cannot stop aging. Nor should you want to. But you can reduce unnecessary metabolic and environmental injury while supporting the skin's natural ability to defend, rebuild and repair. That has been the foundation of my approach to skin for more than 35 years: as a plastic surgeon operating beneath it, as a physician treating it, and as a formulator developing what patients put on top of it.

Protect what you have. Support what your skin can rebuild. And minimize the injury you then expect skincare to repair. That is a far more useful approach to healthy aging than fearing the occasional piece of chocolate.

Focus on long-term metabolic health rather than eliminating individual foods. Limit added sugars and refined carbohydrates, eat a nutrient-dense diet, exercise regularly, avoid smoking, protect against UV exposure and use evidence-based skincare consistently.
